Creekside Dental Patient Information

Creekside Dental is open Monday through Friday from 8 a.m. until 4 p.m. Appointments can be scheduled during these hours by calling (903) 894-6064. If you are a new patient, please read the information below before calling because it may answer some of your questions.     


We will schedule your appointment as promptly as possible, but if you have pain or an emergency situation, every attempt will be made to see you that day. New patients will be required to complete a Health History & Registration form and to sign the Consent section of the form,  Patient Disclosure Instructions and HIPAA forms. For your convenience, these forms are available from the links below. You can print a blank form to fill out by hand. Please complete the forms prior to your first appointment and, if possible, fax the completed forms to (903) 894-6057. We try our best to stay on schedule to minimize your waiting. Due to the fact that we provide many types of dental services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience. Please call (903) 894-6064 with any questions or to schedule an appointment. Once it is scheduled, your appointment time has been reserved specifically for you. We are aware that unforeseen events sometimes require rescheduling an appointment. We request 24-hours notice if you need to cancel your appointment.

For your convenience we accept cash, checks, Visa®, MasterCard®, American Express® and Discover®. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at (903) 894-6064. Many times, a simple telephone call will clear any misunderstandings. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.  We are pleased to offer financing options from Care Credit and Lending Club Patient Financing.  Both Care Credit®  and Lending Club have  programs that offer interest-free financing for up to 18 months and longer terms at low interest with approved credit.         


Your dental plan is a form of compensation provided by your employer. You can expect the carrier (insurance company) to reimburse you for a portion of our fee. That portion is determined by the contract between your employer and the insurance company. The higher the premium paid by your company, the more generous the reimbursement. Although we are not a party to the contractual arrangement with your insurance company, we do want to help you receive the maximum reimbursement to which you are entitled. As a convenience to you we will help you process your insurance claims in order for you to receive this maximum benefit. We will also gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services provided. We will be glad to analyze your insurance benefits for you, but due to the complexities of insurance policies, we can only estimate your coverage in good faith but cannot guarantee your coverage. At all times, you can be confident that we will always provide you with our best services without regard to the limitations imposed by your insurance coverage.
