Creekside Dental is open Monday through Friday from 8 a.m. until 4 p.m. Appointments can be scheduled during these hours by calling (903) 894-6064. If you are a new patient, please read the information below before calling because it may answer some of your questions.
Appointments
We will schedule your appointment as promptly as possible, but if you have pain or an emergency situation, every attempt will be made to see you that day. New patients will be required to complete our new patient packet. For your convenience, these forms are available under patient information. You can print a blank form to fill out by hand. Please complete the forms prior to your first appointment and, if possible, fax the completed forms to (903) 894-6057.
We try our best to stay on schedule to minimize your waiting. Due to the fact that we provide many types of dental services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience. Please call (903) 894-6064 with any questions or to schedule an appointment.
Once it is scheduled, your appointment time has been reserved specifically for you. We are aware that unforeseen events sometimes require rescheduling an appointment. We request 24-hours notice if you need to cancel your appointment.
